We are seeking a dedicated provider with a passion for building meaningful relationships and providing compassionate care to assisted living patients. This role offers flexible hours, high compensation potential, and an opportunity to make a lasting impact through empathetic patient care.

Key Responsibilities:

  • Build and maintain strong relationships with patients, their families, and facility staff, fostering trust and collaboration.
  • Provide compassionate care to assisted living patients, addressing their physical, emotional, and social needs.
  • Communicate effectively and empathetically with patients and their families to ensure understanding of treatment plans and care goals.
  • Collaborate closely with facility staff to ensure seamless care and support for patients.
  • Maintain a flexible schedule, balancing patient visits with some in-office hours (ideally every other Friday from 9 AM - 1 PM).
  • Apply hospice or home health experience to deliver exceptional care to patients requiring complex medical attention.

Qualifications:

  • Licensed Nurse Practitioner or Physician A ssistant with 3+ years' clinical experience, preferably in home health or hospice care.
  • Master's degree from an AMA accredited school.
  • Active, non-probationary, unrestricted Georgia State PA or NP License
  • Active unencumbered DEA License for Georgia OR eligibility to obtain
  • Ability to work in North Metro Atlanta area (Duluth office/facilities in Woodstock, Cumming,, Canton).
