Full Time 40 hours per week (Monday-Friday)

• *No on call, no nights or weekends, flexible 4-day schedule available after first 6 months**

The Home Care Registered Nurse conducts clinical assessments and eligibility determinations to assist the case management team in providing long term services and supports to older adults living in the community. Conducts home visits, nursing facility visits, and medical record reviews and documents determinations according to regulation. The Home Care Registered Nurse also assesses appropriateness for Personal Care and/or Home Health Aide level of care, and develops personal care plans in accordance with Executive Office of Elder Affairs (EOEA) guidelines.

• *Qualifications:**
• Bachelor of Science in Nursing (BSN) preferred; or an Associates of Science degree in Nursing with at least one (1) year clinical experience and/or nursing in the community or long term care setting; or a diploma RN and two (2) additional years of community based nursing experience or long term care direct service experience
• Must possess a valid Massachusetts license to practice as a registered nurse
• Three (3) + years of clinical nursing experience, with at least one (1) year in community or home health preferred
• Proficient in the use of computer systems.
• Must have reliable transportation and valid driver's license.
• Bilingual preferred
