Transformation House is licensed by the Minnesota Department of Human Services, we are a 245G Substance Use Disorder Treatment Facility. We have three residential sites, in Anoka and Fridley, and we have two sober living facilities, in Champlin and Fridley.

Description


Transformation House is lookiing for a Psychiatric Nurse Practioner.

• Make diagnoses
• Prescribe psych meds

• This is a remote position.
• Can be part time employee or contracted
• Approximately 15-20 hours per week

$85/hour

Licensed in Minnesota

Pay Range
$85 per hour
