Position Summary:
We are seeking a Regional Physician Lead to play a key role in providing clinical leadership and oversight for nursing home residents. This position will involve conducting regular Initiating Patient Visits (IPVs) to assess and confirm chronic medical conditions, educate patients on available services, and obtain consent for enrollment in Chronic Care Management (CCM) and Remote Physiological Monitoring (RPM) services. The Regional Physician Lead will also collaborate with a multistate clinical team to support and guide Advanced Practice Providers (APPs) working in nursing home settings. This is a dynamic role that combines clinical leadership with a focus on quality assurance, operational support, and patient outcomes.
Key Responsibilities:
Initiating Patient Visits (IPVs):
Build and establish patient relationships for our services, serving as the billing practitioner.
Conduct approximately 300 clinical visits per month, either onsite or via telehealth.
Document patient encounters accurately and comprehensively within the platform.
Collaborative Oversight:
Provide Clinical Collaborative Practice Agreements as needed for Advanced Practice Providers (APPs) in your region, ensuring compliance with state regulations.
Take responsibility for billing CCM and RPM services under your license number.
Clinical Quality:
Review patient charts, medical records, and treatment plans for accuracy and completeness.
Lead the quality assurance process for CCM and RPM services, including monthly report sampling.
Offer guidance and constructive feedback to APPs to enhance clinical skills and patient outcomes.
Serve as the final reviewer for regional CCM reports and ensure billing accuracy.
Collaborate with the regional team to track key performance indicators, measure patient outcomes, and identify opportunities for improvement.
Cross-Functional Collaboration:
Act as the primary clinical liaison with facility-based medical teams to support the adoption and implementation of CCM services.
Work closely with the business development team to educate clients, support leadership teams, and promote services during new client launches or success meetings.
Other Responsibilities:
Participate in quarterly clinical governance boards to address quality issues or any necessary disciplinary actions.
Perform additional duties and responsibilities as assigned.
Minimum Requirements:
Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO).
Active, unrestricted medical license in the assigned region.
Strong clinical judgment and decision-making capabilities.
Ability to work independently and remotely as part of a multidisciplinary team.
Commitment to providing patient-centered care and improving health outcomes.
Familiarity with Electronic Health Record (EHR) systems and healthcare technologies.
Proficiency with Microsoft Office Suite.
Willingness to travel within the designated region as needed.
