Job Description

Expert Healthcare is looking for a compassionate and dedicated Home Health LPN to join our team of healthcare professionals. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort, well-being, and the effective management of their health conditions. As a Home Health LPN, you will assist with monitoring patients' vital signs, administering medications, and implementing care plans developed by registered nurses or physicians. Schedule: Monday - Friday, No Weekends, No Call required.

Key Responsibilities

  • Provide nursing care to patients in their homes according to established care plans.
  • Administer medications, treatments, and therapies as prescribed.
  • Monitor and record patients' health status and report any changes to the supervising RN or physician.
  • Assist with basic patient care tasks such as bathing, dressing, and mobility.
  • Educate patients and families about medical conditions, medications, and care techniques.
  • Coordinate with healthcare team members to ensure a comprehensive approach to patient care.
  • Maintain accurate and up-to-date documentation of care provided.

Requirements

  • Current and valid LPN license in the state of practice.
  • Experience with HomeCare Homebase Charting preferred.
  • Minimum of 1 year of nursing experience in a home health setting within the last 3 years.
  • Strong clinical skills and the ability to assess patient needs effectively.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time efficiently.
  • BLS certification required.
